Day Care is nurse led and patient centred. We provide an individual programme of care for patients with advanced disease, which is producing symptoms and require specialist palliative care.

Care addresses difficult physical, emotional/psychological and social needs. It also provides valuable respite for families/carers.

The discharge policy involves a detailed nursing and medical assessment of each patient. The patient is reviewed every two months. When the patient's condition is stable and issues are resolved they are discharged from Day Care.

Referrals can be made by any health professional by completing the standard referral form which can be obtained from the Hospice or GP practices. Referrals are processed quickly and patients are contacted within two weeks.
