Update
105 members of staff from the 35 care homes registered with the programme have attended the ‘Introduction to the Six Steps’ half-day workshop.
The session covered the following topics:
- Why End of Life Care is so important
- The importance of talking about death and planning for the end of life
- What a ‘Good Death’ may be from the point of view of the resident/family/care home worker
- The definition of End of Life Care
- National, regional and local drivers of End of Life Care
- The North West End of Life Care Model
- The roles and responsibilities of care home representatives on the course
- Change management
- The audit cycle
- Overview of the Six Steps to Success programme
Actions the staff took away were:
- Develop an End of Life Care Philosophy to be applied to the home
- Quality markers audit (to monitor how End of Life Care in the care home fits with the national Quality Markers and Measures)
- Post-Death Information Audit (to review and document the provision of End of Life care in the home)
- Feedback to members of the wider team
- Commence development of an End of Life Care Policy
- Collate evidence for Six Steps portfolio
