Futures Transition Conference 2025

You are invited to join us at our Futures Transition Conference on 24th September.

This event is free of charge and will be held at Shaw Hill Golf Club.

The conference explores palliative care across all ages with a particular focus on transition from Children and Young Peoples services to Adult palliative care services. The session will showcase the collaborative project undertaken between St Catherines Hospice and Derian House Children’s Hospice.

The agenda for the day is outlined below (may be subject to change) :

0900-0930 – Arrival and refreshments

0930-1015 – Overview of CYP palliative care

1015-1100 – Table discussion 1 – Potential challenges/barriers to delivering effective transition services

1100-1115 – Break

1115-1200 – Overview of Adult palliative care

1200-1245 – Table discussion 2 – Ideas for how you think a transition clinical service could look

1245-1330 – Lunch

1330-1415 – Lived experience

1415-1430 – Break

1430-1630 – Exploration of Futures Transition project – Scoping, service user feedback, education model, physical environment, Clinical pathways and data

1630-1645 – Thanks, final questions

We will explore collaboratively how services can be evolved and developed to ensure patients and families receive timely, compassionate, and coordinated support as they transition between services. This is a really important subject and we’re keen to hear a wide range of views and perspectives, and are able to offer attendance free of change to increase access.
