Six Steps To Success End of Life Care Programme
St Catherine's Hospice has been commissioned by the Cumbria and Lancashire End of Life Care Network to deliver the Six Steps To Success programme to all care homes in Central Lancashire regardless of star rating and at no financial cost.
The Six Steps To Success End Of Life Care Programme has been developed in the North West to support organisational change and staff development to enhance end of life care within care homes. The programme aims to ensure all residents receive high quality end of life care provided by a care home that encompasses the philosophy of palliative care.
Programme Content
The programme consists of eight workshops which should be attended by a nominated care home representative and two mandatory education workshops which are open to all staff - the Liverpool Care Pathway, Communication Skills, Advanced Care Planning, Mental Capacity act, Advanced Decisions To Refuse Treatment, Do Not Attempt Resuscitation, Lasting Power Of Attorney and Best Interest Decisions. The core programme is based on the six steps described in the 'Route to Success: a guide to improving end of life care in care homes' (2010):
Step 1: Discussions as the end of life approaches
Step 2: Assessment, care planning and review
Step 3: Co-ordination of care
Step 4: Delivery of high quality care in care homes
Step 5: Care in the last days of life
Step 6: Care after death
Care home staff will be supported to develop their knowledge and skills and will be encouraged to empower colleagues to deliver high quality end of life care. The programme will support collaborative, multiprofessional working across all settings, enhancing end of life care for residents and families. The programme will promote a skilled and motivated workforce, enabling residents to die in their preferred place of care and avoid unnecessary hospital admissions.
